Keynote Friday, 8:05 am – 9:25 am
Do No Harm and Tell No Lies: Using Evidence-Based Medicine to Optimize Patient Safety and Promote Mainstream Medical Community Acceptance.
1Abstract
The awareness of ankyloglossia in mainstream and social media has not been matched with widespread awareness in the medical field. Despite an increase in the number of studies devoted to exploring the impact of ankyloglossia and other oral ties, there is still a rift between who does and doesn’t offer frenotomy. As a result, mother/baby dyads may not be successful in meeting their feeding goals.
As more professionals become aware of the gap in medical understanding, more professionals are becoming involved in the management of dyad feeding problems in an attempt to fill those gaps. Often, the decision-making paradigm surrounding oral ties is based more on gestalt rather than existing published studies. Whether it is from a lack of awareness of what is currently published or a lack of emphasis on the need to practice evidence-based medicine, the resulting decision to perform a frenotomy may be viewed with negativity by other professionals.
This talk aims to demonstrate the importance of practicing evidence-based medicine and, more importantly, how to give weight to what is and isn’t published in an attempt to enhance patient safety, improve patient outcomes, and foster acceptance of the science in the medical community.
1Biographical Highlights

My Sleep Disorder: Taking My 10-Year Journey to Your Patients.
1Abstract
Dental sleep medicine has significantly advanced over the last ten years. Initially, there were few tools beyond mandibular advancement appliances (MAD) to help adult patients and even fewer for children. That has now changed with the advent of early intervention orthodontics, myofunctional therapy, and various laser treatments. I aim to share my own personal experiences in overcoming my sleep apnea. I have sought out many of these current treatments to help manage my sleep disorder. Through my journey I am better able to connect the dots from diagnosis to treatment options when managing the adult patient. Yet I feel we need to continue to push current medical approaches and look even deeper and ask more questions on how best to help our patients improve their health and quality of life.

Creating Urgency for Airway-Centric Dental Care.
1Abstract
Airway health is a hot topic in dentistry and is something that all Dentists need to be aware of. We have the unique perspective of treating patients of all ages and identifying sleep-disordered breathing (SDB) and obstructive sleep apnea (OSA) warning signs and symptoms. It is our responsibility to ask more questions and see beyond the teeth of our patients. In this course, we will discuss the signs and symptoms to look for and give you easy checklists and action items to arm you with the ability to make a difference in the health of your patients’ lives on Monday when you return to your office. We will also discuss how undiagnosed SDB and OSA can wreak havoc on restorative dentistry and lead to an increased risk of failures and breakdowns if not identified prior to treatment. You will gain a deeper understanding of the “why” behind many common frustrating problems we see every day, such as fractured teeth, acid erosion, abfractions, crowded teeth, tooth wear, temporomandibular disorders and anterior open bites.
Case studies will be provided to show you what is possible in interceptive orthodontics to treat SDB in children as well as what you can do for your fully grown adult patients who suffer from snoring and/or OSA using orthodontic appliances and myofunctional therapy. You will be shown that it is possible to expand the palates of patients of all ages to prevent the need for premolar extractions and make more room for the tongue in your patients’ mouths so that it is no longer obstructing airways and impairing breathing.
Take your practice to the next level by solving problems that others do not even know exist while helping your patients live longer and healthier lives by looking beyond their teeth.

Functional Effects of Tethered Oral Tissues Across the Life Span: When and Why to Release.
1Abstract
Tethered oral tissues can have a profound impact on oro-facial functioning throughout the life span. Correctly diagnosing this functional disorder and determining the optimal timing for release requires complex decision-making and a team approach that includes dental, medical, and therapeutic professionals. Releasing the tongue at the wrong time, whether the issues are underlying pre-requisite orofacial skill development, structural constraints, or psycho-social factors, can do more harm than good. We need to look at the whole child/patient in the context of the family and community and ask the right questions to help guide the recommendations and empower families to make the best decisions for their loved ones.

Foundations for Successful Functional Frenuloplasty in the Child and Adolescent Population.
1Abstract
Appropriate tongue space, function, timing, and compliance are common factors/challenges when planning for a frenectomy or functional frenuloplasty. When evaluating and treating children and adolescents for tethered oral tissues, adequate tongue space and post-operative compliance can be challenging and prevent us from having favorable outcomes. Release providers often are faced with the challenge of either placing sutures or instructing caregivers to do wound healing stretches on their own. Case studies and knowledge from previous research have helped us formulate key factors that can help lead to positive outcomes for the patient and provider.

A Simple Cut, Profound Relief: The Broad Benefits of Adult Frenulum Release: A Case Study.
1Abstract
This case study explores the immediate systemic benefits of adult lingual frenulotomy. Lingual frenulotomy is a surgical procedure that involves the incision of the frenulum, a small fold of tissue that may restrict movement of the tongue. It can be performed using manual surgical instruments or lasers, with laser techniques offering advantages such as precision, reduced procedure time, minimal bleeding, and faster recovery.
Laurie, a 58-year-old female, reported waking daily around 3 a.m. with severe headaches (Headache Impact Test [HIT-6] = 72), neck and head tension mainly in the front and rear left of the head, disruptive snoring, and poorly managed hypertension. Two weeks after the lingual frenulotomy, Laurie reported no headaches for ten days, cessation of snoring (as confirmed by her spouse), improved sleep (as indicated by the return of dreams), and a significant reduction in neck and shoulder tension. Remarkably, she also demonstrated a consistent decrease in her daily blood pressure readings, which she reported as being lower than they had been in years. One month after the procedure, Laurie reported a HIT-6 score of 36.

Dr. Hill subsequently practiced general dentistry for 20 years in Sylvan Lake, but since 2020 he has taken an interest in the impact and treatment of tethered oral tissues in infants, children, and adults.
2Biographical Highlights - Barbara Brown, RDH, COM®
Barbara Brown has been a registered dental hygienist for 25 years. After graduating, Barb worked in Switzerland for just under 3 years and was first introduced to the European Philosophy of complete patient care which she still utilizes today in her practice.
She began her Orofacial Myofunctional journey by taking an intro course in Calgary, Alberta in 2012. Her main intent for attending the course was to help her dentist in the prevention of orthodontic relapse. She completed her certification in 2015 thru the IAOM. After many years of continuing education courses, she has since turned her main focus and passion to promoting overall orofacial health and improved well-being through myofunctional therapy. She has worked very hard in establishing a multidisciplinary approach to her patient’s treatment plans.
Barbara opened her independent home based practice, My Myo Smiles in Airdrie, Alberta in 2018. It was days before opening her own practice that she decided to do an evaluation/exam (to check the flow of the office) on her older child and had an overwhelming realization that he was in fact tongue tied. Looking back at the history she (and her husband) had with her son regarding breast feeding difficulties, asthma, allergies, speech and sleeping issues, she then decided to help as many families as she could, hoping they would not experience the same challenges. She believes educating those around her that Orofacial Myology is the missing link to complete patient care, whether it be through social media, lunch and learns, parent group discussions, etc. Spreading awareness is KEY !!!

The Tongue Tie Spectrum.
1Abstract
Tongue tie is described as a congenital condition that can impact functions such as feeding and speech. We see many patients and families present with patterns of symptoms that would suggest that there are conditions that frequently co-exist with tongue tie, so the concept of the tongue tie spectrum or tongue tie complex is a phenomenon. This presentation will explain what this is and start the discussion on why. We also look at adaptations needed during the treatment process to optimize their care.

It Is Not Just the Frenum: How to Think Outside of the Frenum Box to Get your Best Results.
1Abstract
To achieve the best results in laser frenectomy procedures, it is essential to consider factors beyond the physical appearance of the frenum. Assessing various aspects of the birthing process—such as fetal positioning, umbilical cord complications, dystocia, genetics, muscle tone, head shape, and the alignment of the head, neck, shoulders, and hips—allows us to ensure the body functions holistically. Understanding the impact of pacifier use and other external interferences also contributes to more consistent and effective outcomes. Focusing solely on the healing aspect leads to a narrow approach that may not fully serve patients of any age.
In this presentation, I will delve into numerous factors that enhance our diagnostic capabilities and clarify the observations we encounter in clinical practice. We will explore topics like head shape and the use of helmets in correcting cranial abnormalities and examine laryngomalacia to understand its effects on airway function. The discussion will include how eye position and excessive tearing can indicate underlying issues, as well as the significance of toe walking in relation to developmental or neurological conditions. We will assess the impact of swaddling practices, devices like “Snoos,” and sleep habits on child development and investigate how pacifier use influences healing processes.
Additionally, I will discuss the relationships between headaches, sleep disturbances, seizures, and sleep-disordered breathing (SDB). The presentation will address airway challenges associated with conditions like achondroplasia and examine the implications of connective tissue disorders such as Ehlers-Danlos Syndrome on healing. By broadening our diagnostic perspective to include these factors, we can better understand the complexities of each patient’s condition and provide more comprehensive care.

Improving Outcomes with Release: A Discussion of Reattachment and Root Cause of Unexpected Outcomes.
1Abstract
In the wake of negative press surrounding the release of tethered oral tissue, it is necessary for practitioners to take a step back to reassess our processes and address the controversy. Negative press focuses on unexpected outcomes of the release process. Lack of improved feeding, regression in feeding, infant pain and suffering, and poor wound healing are some of the unanticipated results seen when the release process does not go as expected. This lecture looks at root cause analysis of unforeseen outcomes with case studies, providing feedback and analysis. Through introspection, thoughtful discussion, and collaboration, we will continue to improve our outcomes and dispel controversy surrounding our profession.

Ties & Time: A Compilation of Cases from 0-70 Years Old.
1Abstract
Ankyloglossia and other restrictive oral tissues have been a contentious topic of debate among different healthcare professionals for many years. Interest in what constitutes an oral restriction and when it should be addressed surgically has been on the rise in recent years. The renewed interest has, in part, been driven by the general public seeking answers and solutions to problems the traditional health system has overlooked. The wide accessibility to free online resources, podcasts, and social media has allowed laypeople, especially parents and patients themselves, to start connecting the dots and seek more specialized advice and care.
This presentation is designed to show the impact of oral restrictions over a lifetime. It is a compilation of cases from different ages and backgrounds with a thread of commonality in signs and symptoms, uniting them to get a broader perspective on how oral restrictions can impact growth, development, and quality of life. The sample consists of 700+ patients ranging in age from 1 day old to 70 years old. The sample was selected to undergo a frenectomy or frenuloplasty procedure based on self-reported signs and symptoms using Dr. Baxter’s assessment form, followed by a clinical exam by a frenectomy provider and a supporting team of IBCLCs for infants and SLP or myofunctional therapist for older patients. The results were derived and compiled from a self-reported follow-up assessment questionnaire.
Conclusions from the report show a positive correlation between frenectomy/frenuloplasty procedure and self-reported improvements in signs and symptoms of their initial concerns. The results presented do have limitations, including reporting bias, leading questions on follow-up assessment, and lack of follow-up on some patients who received the procedure but did not return for follow-up.
Limitations notwithstanding, the overarching goal of the presentation is to highlight real-life case scenarios over a lifetime and to increase awareness of a poorly understood congenital malformation that could be the underlying cause of a plethora of other medical and psychological conditions impacting patients’ and families’ quality of life.

I am a practicing pediatric dentist and my educational training includes an undergraduate degree from the University of South Florida in 2005. Dental training from Nova Southeastern University in 2009 ,followed by an AEGD program from University of Florida in 2010 and pediatric dentistry certificate from UCLA in 2012.
I was a board-certified pediatric dentist for eight years prior to recognizing I was missing some basic understanding of tongue function, its impacts on growth and development, and overall quality of life. In 2020, I fell asleep with my 3-year-old daughter, the entire night I was not able to sleep due to her constant teeth grinding, tossing and turning and gasping for air. The next morning, I woke up tired but motivated to find out what exactly was going on with her during sleep. It was not anything I had learned about in my formal training. That restless night planted the seeds for an unexpected journey of learning, collaboration, and questioning. In the past five years, I have taken many courses to advance my understanding of tongue, lip, and cheek ties and their impact on daily life. During the learning process, it became apparent that even among experts on the subject, there were still questions and differences of opinion as to when one should or should avoid performing a release procedure. During this time, I performed procedures on patients from birth to 70 years old.

Form Follows Function in Children’s Orofacial Growth.
1Abstract
The body has an amazing capability to adapt to survive. When it comes to the development of orofacial structures, there is a synergistic effect between form and function. The tongue plays an important role in a child’s orofacial growth and development. The tongue’s ability to move freely in the mouth and rest comfortably against the palate can have a direct effect on other basic functions like sucking, chewing, swallowing, speaking, and breathing. Dental issues like a narrow palate, cross-bites, crowding, gingivitis, dental cavities, and poor orofacial growth may also develop as a result of improper tongue posture.
There are several factors that can contribute to abnormal tongue posture and function. Recognizing these factors in early childhood may allow us to intervene at the right time to correct function and maximize growth potential. Adequate screening seems prudent when evaluating children with apparent tongue dysfunctions. Treatment goals should be geared towards restoring structure and function. A team approach with other specialists may be necessary to help the patient overcome their compensations and promote proper orofacial growth.

Chiropractic Collaborative Approach to Tethered Oral Tissues and Airway Dysfunction.
1Abstract
As we learn more about tethered oral tissues and their effects on the body, chiropractors play an important role in helping identify, treat, and educate families about the implications of TOTs on the body and possible long-term implications leading to disordered breathing. With our collaborative approach, we are better able to identify if intervention is needed at an early age. In this course, providers will learn how chiropractors identify tethered oral tissues in practice, along with referral recommendations to frenectomy providers. You will also learn the benefits of chiropractic care pre and post-release, the benefits of chiropractic care during expansion, and how this care leads to overall better outcomes for the patients. This course is designed to help frenectomy providers understand the importance and benefits of chiropractic care as a collaborative approach to better serve this patient population.
